About the Role The Occupational Therapy Assistant will work closely with the Occupational Therapists and the interdisciplinary team to help our seniors maintain and improve their independence, safety, and ability to perform daily activities. They will provide therapeutic interventions, monitor and document participant progress, educate participants and caregivers on adaptive strategies and equipment, support care planning, and collaborate across the care Responsibilities Implement occupational therapy treatment plans under the supervision of a licensed Occupational Therapist. Deliver therapeutic interventions that support participants' independence, safety, mobility, cognitive function, and ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Reinforce therapy recommendations and support participants in developing, maintaining, and restoring functional abilities that promote aging in place and community living. Observe participant progress and response to treatment, documenting findings and communicating relevant clinical information to the Occupational Therapist and Interdisciplinary Team (IDT). Assist with functional assessments, care planning activities, and identification of participant needs, risks, and opportunities for intervention. Educate participants, caregivers, and care team members on therapeutic techniques, adaptive strategies, home safety recommendations, and the appropriate use of adaptive equipment and assistive technology. Complete timely and accurate documentation within the electronic medical record (EMR) in accordance with organizational, regulatory, and professional standards. Collaborate with the Interdisciplinary Team (IDT) to coordinate participant care, contribute to care plan development, and support positive participant outcomes. Participate in team meetings, quality improvement initiatives, and other program activities that advance Seen Health's mission and goals. Utilize technology and digital tools to support care coordination, documentation, participant outcomes, and operational excellence. Assist with ordering, tracking, and maintaining  therapy equipment and treatment areas, respond appropriately to participant incidents or emergencies, and perform other duties as assigned.
