InCare is seeking a full-time Nurse Practitioner or Physician Assistant to join our Hospital Medicine team caring for patients in an inpatient acute rehabilitation setting . This is an excellent opportunity for an APP who enjoys managing medically complex patients while working closely with an interdisciplinary team to help patients recover and regain independence. Job Responsibilities: Responsibilities: Perform admissions, daily follow-up visits, and discharge evaluations Manage acute and chronic medical conditions in an inpatient rehabilitation setting Collaborate with physicians, nurses, therapists, pharmacists, and case managers to develop individualized treatment plans Participate in interdisciplinary team meetings Document patient encounters accurately and timely in the electronic medical record Provide compassionate, evidence-based care focused on improving patient outcomes and facilitating successful rehabilitation Qualifications: Qualifications: Current Nurse Practitioner or Physician Assistant license DEA registration (or ability to obtain) Experience in hospital medicine, internal medicine, rehabilitation medicine, or a related field preferred Strong clinical assessment, communication, and critical thinking skills Ability to work independently while collaborating with a multidisciplinary team Commitment to providing exceptional patient care Working Hours: 8AM-5PM
