RN Case Manager – Hospice | Full-Time | Albany, OR 💰 $49.00 – $55.00/hour (DOE) + Benefits 🌿 Deliver Expert Care Where Patients Feel Best—At Home Signature Albany Hospice is seeking a skilled and compassionate Registered Nurse (RN) Case Manager to provide high-quality Hospice care to patients in the comfort of their homes. This full-time role is ideal for an RN with experience in Hospice , H ome Health, care coordination, and patient-centered care , who thrives in an independent, field-based environment while collaborating with an interdisciplinary team. 🔎 What You’ll Do Patient Care & Case Management • Perform comprehensive nursing assessments and develop individualized plans of care • Coordinate and manage hospice services in collaboration with physicians and care teams • Provide skilled nursing care, treatments, and patient education • Monitor patient progress and communicate changes in condition to physicians • Help ensure high-quality, compassionate hospice services are delivered consistently.
