InCare is seeking a full-time Nurse Practitioner or Physician Assistant to join our Hospital Medicine team at Encompass Rehabilitation Hospital in Concord, New Hampshire . This position offers the opportunity to practice in a collaborative inpatient rehabilitation setting, caring for medically complex patients while working alongside an experienced team of physicians, APPs, nurses, therapists, and case managers. Job Responsibilities: Responsibilities: Perform daily patient evaluations and follow-up visits Manage acute and chronic medical conditions in the inpatient rehabilitation setting Collaborate with physicians, nursing staff, therapists, and case managers to develop and implement treatment plans Participate in interdisciplinary team meetings Document patient encounters accurately and timely in the electronic medical record Promote high-quality, patient-centered care and positive patient outcomes Qualifications: Current Nurse Practitioner or Physician Assistant license in New Hampshire (or ability to obtain) DEA license (or ability to obtain) Experience in hospital medicine, internal medicine, rehabilitation medicine, critical care, or a related specialty preferred Strong clinical assessment and communication skills Ability to work independently while collaborating within a multidisciplinary team Commitment to compassionate, high-quality patient care Working Hours: 8:00 AM to 5:00 PM Skills: Required Skills: Strong clinical assessment and diagnostic skills Ability to manage acute and chronic medical conditions Knowledge of internal medicine and hospital-based care Excellent clinical decision-making and critical thinking abilities Ability to prioritize and manage multiple patients effectively Strong communication and interpersonal skills Ability to work independently while collaborating with physicians and interdisciplinary teams Proficiency with electronic medical records (EMR) Commitment to high-quality, patient-centered care Strong documentation and time-management skills Preferred Skills: Experience in hospital medicine, internal medicine, rehabilitation medicine, critical care, or skilled nursing facilities Familiarity with post-acute and rehabilitation patient populations Experience coordinating care with therapists, nursing staff, case managers, and specialists Understanding of quality metrics, patient safety, and discharge planning
