Position Overview Under the direction of the Director of Primary Care, provides services to psychosocial evaluation, crisis and trauma counseling, resource referral and case management services. Accountable for intervening with patients and families, by applying critical thinking skills to monitor psychosocial status and issues that impact the clinical progression an transition/discharge plan for patients, including facilitation of patient and family communication, coping and decision making. Works as part of an interdisciplinary team. Primary Department, Division, or Unit: UHS Primary Care Walton Work Shift and Schedule: This is a per diem position, which means you will work on an as needed, agreed upon basis. Working hours will be assigned by your manager. Compensation Range: $33.63 - $50.45 per hour, depending on experience This position is not eligible for benefits. ----- Education and Experience Minimum Required: • Master’s degree in social work from an accredited program with licensure as Licensed Clinical Social Worker (LCSW) in New York State. • Experience working with specific populations, such as individuals with mental health concerns, families, or older adults. • Experience with electronic health records, ability to work independently and as part of a team and commitment to ethical and culturally sensitive practice. Preferred: • Preferred 3 years of postgraduate experience.
